Learn the History
The Coalition was formed in 2011 under the leadership of Prince George's County Health Department (PGCHD). The original Community Health Improvement Plan focused on ensuring access to superior health care. In 2016 PGCHD and all five area hospitals led the process to update the Community Health Needs Assessment. The Assessment determined additional areas of focus: Behavioral Health, the Social Determinants of Health, Obesity and Metabolic Syndrome, and Cancer.

About the Coalition: Health is a shared responsibility, and our 400+ members come from many parts of our community with different backgrounds and skill sets that help us work together to elevate the health and well-being of all Prince George's County residents. We include members from departments and industries like health and human services, healthcare, transportation, parks and recreation, nonprofits, education, and more. Our approach integrates health into the framework of our daily lives as we work toward creating and nurturing an environment where the healthy choice is the easy choice in our communities.
In 2016, The Prince George's County Health Department along with all area hospitals led the PGHAC process to update the Community Health Needs Assessment. A select Community Advisory Group representing populations and agencies across the County reviewed all the information collected and prioritized these PGHAC focus areas:

- Obesity and Metabolic Syndrome and
- Cancer
